Solution
Pass Visits was checked in the Global Waiver Settings within Static Parameters which is incorrect.
Steps for Solution
- Go to Management > System Management > Profile Assignments.
- Static Parameters Button.
- Expand Global Waiver Section.
- Turn off pass visits from Global Waiver Modules.
- Click Save.
