The CYMS Reports program allows you to run various Child Youth Management System reports involving medical conditions and shots.
- CYMS HH/FM Profile Print - Allows you to print select information for the households and family members.
- CYMS Medical Report - Allows you to report on Medical Issues, Medication, Allergy, Illness, Basic Care, and Special Needs information for children by Activity or by Pass Type. Data for this output is pulled from the Medical, Allergies, and Illnesses tabs in Household Update • Member Details.
- CYMS RecConnect - Allows you to generate a list of Family Members who meet your report criteria. From there you can send emails, run reports and labels. You can also export the list to a .csv file, if desired.
- CYMS Shot Report - Allows you to report on shots that have been given and shots that are due or past due for a selected range of Shot Codes, Due Dates, and Family Members.
RecTrac Process and Reporting screens offer numerous fields and widgets that allow you to filter the data for which your process or report is being run. RecTrac Widgets provides definitions and examples to show you how these work.
From the bottom of the screen…
Custom Title - Enter a free form text in this field, if desired. The text you enter here will overwrite the default report title. Leave this field blank to accept the default title.
Output Template - Accept the default entry or expand the Drop-down list to select an alternate Template, if desired. The Templates displayed are the reports that can be run.
"VSI -" Templates are RecTrac standard reports. Report Output Templates are maintained in Report Output Management.

Section Criteria
Sections (ARSection_ComboKey_DataGrid)
The Activity Section DataGrid displays all sections in your database based on the DataGrid filters being used and your Permissions.
Highlight/select individual Sections, as needed, from the DataGrid. Multiple selection is allowed.

The functionality of this field differs depending on whether you are running a Report or a Process.

Select the Shot Code for which you want to create or delete records when running this process. This is a Mandatory field for the default Vermont Systems design. Your design and setup may vary.

This field is applicable only if you opt to Use Shot Range.

Note: Waived shots are excluded from this option regardless of whether they have a shot date or not. To include waived shots on your report select/enable the Print Waived Shots option.
Include Shot Comments (CYMSReport_PrintComments)
Select this option to have any text in the Shot Comments field print on your report. Shot Comments can be written when adding or updating a shot record to a family member.
DeleteInclude Waived Shots (CYMSReport_PrintWaived)
Select this option to have waived shots included on your report output. Waived shots are excluded from the Show Option field, so you want them included, then you must select this option.
DeleteInclude Shots Without Due Dates (CYMSReport_PrintWODueDate)
Select this option to have any shots without a due date included on your report. For this filter to work, the shot has to fall within your other ranges (i.e. you cannot print a report just for shot records without due dates).
DeletePrint Contact Information (CYMSReport_PrintContactInfo)
Select this if you want to print contact information for the child's sponsor. This will add a new row under each member on the report with their contact information. Contact information will include Primary Sponsor Name, Primary Phone, and Primary Email Address.
DeleteMedical Criteria
Medical Codes (SAMedicalCode_RecordCode_DataGrid)
Select individual Medical Codes or a range of Medical Codes from the DataGrid, if desired. The Medical codes you select will be included in the process you are running.

- Detail - The report will list individual family members and their conditions in their own section. Sponsor name and contact information is included.
- Summary - The report will list family members and their conditions only. No sponsor information is included.
Sort Option (CYMSReport_SortOptionMedical)
This field is applicable only if your Report Option (Core Criteria tab) is "Activity" or "Pass."
Select your Sort Option.
- Name - The report will sort alphabetically by last name.
- Room/Pass The report will sort alphabetically or numerically in ascending order by Section Code or Pass Code.
Include (Blank = All) (CYMSReport_IncludeOptionMedical)
Select your Include"ilter options if desired. Multiple selection is allowed. In order to be included on the report the corresponding Medical Codes must be included in your Medical Code Range or DataGrid (or both). For example your Medical Code Range is your Illness Codes only. If you select "Food Allergies" in this field then the report will yield 0 results due to the Medical Code / Include option mismatch.
The exception to this is Special Needs. Special Needs do not appear in the Medical Code Range or DataGrid. In order to report on Special Needs that option must be selected here. To report on Special Needs records make sure "Special Needs" is selected in this field.
Leave this field at its default value to "include all" and have the report include all Medical Codes within your Range and/or DataGrid.
Note: For this field making 0 selections results in the all entries being included. For the Print field making 0 selections results in the field being skipped.
Include Details And Show The Following (CYMSReport_PrintOptionMedical)
Select your Print options, if desired. Multiple selection is allowed.
- Treatments - This option is not currently used.
- Reaction - Select this option to have the text that appears in the Reaction field of the Allergies tab in Household Update • Member Details included on the report.
- Comments - Select this option to have the text that appears in the Comments field of the Medical Conditions, Medications, Basic Care, Allergies, and Illness tab in Household Update • Member Details included on the report.

Print All Records for Child - Select this option to have all instances of a child listed on the report. Disable this option to have the child listed once only. With this option selected, a child will be listed on the report every time the child is found. For example, Loretta Martin has asthma and is enrolled into a Before School program and an After School program. When running this report for your School Age programs, Loretta will be listed:
- Once only with this option disabled
- Twice with this option selected due to her multiple enrollments. (Loretta will be listed once for the Before School enrollment and once for the After School enrollment).

Print Child's Photo- Select this option to have the child's stored .jpg image appear on the report. If a child does not have an image linked to his/her record, then your no-photo.jpg will display on the report output.
Note: Enabling this option will increase the run time for this report significantly.
Leave this field at its default value to skip the printing of these options.
Note: For this field making 0 selections results in the field being skipped. For the Include field making 0 selections results in all entries being included.
New Page Per Room/Pass (CYMSReport_PrintNewPage)
This field is applicable only if your Report Option (Core Criteria tab) is "Activity" or "Pass" and then only if your Sort Option (on this tab) is "Room/Pass."
Select this option to have the report to insert page breaks so that records linked to family members in each Room Code or Pass Code begin on their own page.
Deselect this option to omit the page breaks between Room and Pass Codes.
DeleteSkip Extra Line (CYMSReport_PrintLine)
This option is available only when using the Summary option.
Select this option to omit the carriage returns after each line on the report.
Leave this option deselected to insert carriage returns after each line on the report.
DeleteUse Illness Date Range (CYMSReport_UseIllnessRange)
Select this option to filter the report by family members with an Illness record within a range of dates.

The report looks at both options because a family member can be Special Needs only or Special Needs and EFMP. However a family member should never be EFMP only.

Select this option to filter the report by family members who are missing either a SNRT Initial Date or a SNRT Last Date on their Member Details screen in Household Update. When this option is selected the system filters the report by family members who have the Member has Special Needs? option enabled but have a date missing.

Health Assessment Criteria
Use Health Assessment Criteria (CYMSReport_UseHealthCriteria)
Select this option to filter the report by family members with a health assessment coming due within a range of dates.

Accept the default entry (365) in the Number of Days Health Assessment Good for field or overwrite with another value.

Select a Health Date Option.
When this option is selected, the system filters the report by Health Care Assessments coming due. Only those family members with a health assessment coming due within the date range entered will be considered for the report. Family members who do not have health assessments coming due within the date range entered will not appear on the report regardless of whether they meet other criteria, such as shots coming due. This includes family members whose health assessment is past due as well. In light of this, Vermont Systems recommends entering a begin date that is far in the past (For example, 01/01/2000) and an end date at least six months in the future, if you are opting to filter by health assessment.
For example, today" is 30 June. You are running the report for shots coming due within the next month. You select this option and enter 07/01/20xx - 07/31/20xx in the Health Begin and End Date fields. Ted Lockwood's health assessment is good until October 20xx.
