Select this option to run the report for family members with shots coming due on or before the date entered in the field.

Days for which Physicals are good (ReportOptionsHouseholdReminderReport_CheckPhysicalDays)
Accept the default entry (365) or overwrite with an alternate value. The value you enter here determines whether a family member's physical will be due by the date entered in the Coming Due By field. When you run the report, the system looks at the Last Physical date field for each family member and adds this number of days to it. If the Physical will be expired by the Coming Due Date, then the family member appears on the report.
For Example: Maggie Halsey's Last Physical date is 09/01/20xx. If you enter '365' in this field (1 year) and run the report for sports physicals due to expire on/before 08/31/20xy, then Maggie will NOT be included on the report because the system assumes her physical expires on 09/01/20xy (09/01/20xx + 1 year = 09/01/20xy).
If you enter '181' in this field (approximately 6 months) and enter 08/31/20xy as your 'due by' date, then Maggie will be included on the report because the system assumes her physical (dated 09/01/20xx) expires on 03/01/20xy (09/01/20xx + 6 months = 03/01/20xy).

Select this option to run the report for family members with a health assessment coming due on or before the date entered in the field.

When you run the report, the system looks at the Health Assessment Date field for each family member and adds this number of days to it. If the Health Assessment will be expired by the Coming Due Date, then the family member appears on the report.
For Example: Ted Lockwood's Health Assessment Date is 09/01/20xx. If you enter '365' in this field (1 year) and run the report for health assessments due to expire on/before 08/31/20xy, then Ted will NOT be included on the report because the system assumes his health assessment expires on 09/01/20xy (09/01/20xx + 1 year = 09/01/20xy).
If you enter '181' in this field (approximately 6 months) and enter 08/31/20xy as your 'due by' date, then Ted will be included on the report because the system assumes his health assessment (dated 09/01/20xx) expires on 03/01/20xy (09/01/20xx + 6 months = 03/01/20xy).

Notes: In order to report on Family Care Plan the Family Care Plan Required option on the Sponsor Information screen in Household Update must be enabled. If the Family Care Plan Required option is NOT enabled then no FCP records will be printed for a household regardless of whether you have dates in the Family Care Plan Due Date and/or Signed by Commander date fields.
How the Dates are used on this Report
- IF there is a date in the Family Care Plan Due Date field but NO date in the Family Care Plan Signed Date field then the report will use the Family Care Plan Due Date. If the date entered is prior to the Due by Date entered for this report then the household will print on the report. because the Family Care Plan is past due.
- IF there are dates in both fields then the report will use the Family Care Plan Signed Date since it should always be the most recent date. If using that date then the report will add the Good For Days to the Signed Date to determine whether the Family Care Plan is coming due/past due.
Family Care Plan fields (Due Date Signed Date and Family Care Plan Required?) do not appear on the Standard VSI Design. They can be added using Custom Screen Design and typically are located on a Group in Household Update.

Include Household Ticklers (ReportOptionsHouseholdReminderReport_IncludeTicklers)
Select this option to have any Ticklers linked to a Household printed on the household's report.
Delete1/2 Page Per Person If Possible (ReportOptionsHouseholdReminderReport_HalfPage)
Select this option to limit the report output to a half page per family member per household when possible. This offers a good way to save paper.
Note: This option is applicable only when using the 'Standard' Report Format option.
Email Each Household Their Reminders (ReportOptionsHouseholdReminderReport_SendEmails)
Select this option to send an email to each household that contains their reminders. A .pdf will be generated for households that do not have an email on file.
DeleteComment Code (ReportOptionsHouseholdReminderReport_CommentCode)
Click the Picklist icon to select the Comment Code, if any, that you wish to have printed on the report.
Comment Codes are maintained in Comment Code Management. Type = CYS Report.
DeleteReport Format (ReportOptionsHouseholdReminderReport_ReportFormat)
Expand the Drop-down list to select your Report Format.
- Letter File - Prints a letter using the CYS Letter Code you select.
- Standard - Prints as a standard report for each household. This is the default option.
Letter Code (ReportOptionsHouseholdReminderReport_LetterCode)
This option is applicable only if you are using the "Letter Format" Report Format
Click the Picklist icon to select the Letter Code that you wish to use for this report.
Letter Codes are maintained in Letter/Waiver Management. Type = CYS Info.
DeletePrint Salutation With Rank In Letter (ReportOptionsHouseholdReminderReport_PrintRank)
This option is applicable only if you are using the "Letter Format" Report Format
